Subject access requests

The Data Protection Act gives everyone the right to access personal information that Barnsley Hospital holds about them. We hold data about patients, and people who work here employed by Barnsley Hospital Foundation Trust.

Any request for personal information from us is known as a Subject Access Request.

We will ask for proof of identity before any personal information is released.

Please be aware we reserve the right to charge you a fee for a Subject Access Request if it is deemed to be manifestly unfounded or excessive.

Use our online form to make your request. You need to make sure you include as much detail as possible about the information you need to see so we can find it as quickly as possible.

Making a request using a paper form

If you need to make your request on paper, please download a copy of the paper form, then print, complete, and post your request.

Once you have completed your printed form, please ensure:

  • You have included all necessary documents.
  • You send your message by recorded delivery.
