Patients who are treated within the Intermediate Care Service may now be cared for at Barnsley Hospital as part of this provision.

This means that patients, who need rehabilitating before returning to their homes, can receive the extra support and care necessary to avoid a readmission to hospital or on discharge from hospital before they go home.

The Transition Unit is a very important component of the Intermediate Care Pathway, as a the permanent facility with a patient / carer centred approach to care and support. This will be based on Ward 35 within our hospital and patients will receive short term specialist care for a period of up to 14 days whilst in our care.
