South Yorkshire ICB seeks feedback on Patient Transport Services

South Yorkshire Integrated Care Board (ICB) are undertaking a review of Patient Transport Services. The ICB are seeking feedback from patients who have recently used the service.

Two white mini-buses used for patient transport, to and from Barnsley Hospital - the NHS logo and Yorkshire Ambulance Service is written on the buses.

A spokesperson for the South Yorkshire ICB said: "We ask that you complete this survey on your recent journey to identify the strengths and weaknesses in our service. By letting us know what is important to you, your opinions will influence improvements to your service."

Patients can provide feedback by either filling in an online survey, or completing a Word document version.

Surveys completed via the Word document can be sent back via the online survey link or printed out and returned by post.

Paper copies should be posted (Freepost address) to:
