Our Acorn Unit is for people who need extra support, care and rehabilitation to avoid them having to go into hospital unnecessarily or to support them on discharge from Hospital. It is for patients whose Intermediate Care and rehabilitation cannot be provided in their own home.

Staff on the unit work closely with you, your family/carers, community health and Social Care staff, providing therapeutic assessment and treatment which aims to help you return to an optimal level of independence. People will usually stay on the Unit for approximately 14 days.

You might be referred to the Unit if:

  • You have been unwell, or have been in hospital and may require a further period of rehabilitation before going home or to your usual place of residence
  • You are currently in hospital and have been identified as medically well enough for discharge but require ongoing support
  • You are registered with a Barnsley GP
  • You have been seen by the Hospital or Community therapy/Nursing or Medical team and would benefit from intermediate care intervention

The Acorn Unit is located at Highstone Mews Care Home:
